How TransitionCare Health & Wellness Began
TransitionCare Health & Wellness grew out of the belief that healthcare should be personal, accessible, and unhurried. After years in primary care, specialty neurology, and clinical research, Dr. Noureen Dhanani saw how many adults felt rushed, unheard, or unsure about their next steps, and decided to design a new kind of telehealth practice.
Our Neurology-Informed, Evidence-Based Approach
With a Doctor of Nursing Practice degree and specialization in Neurology, along with Adult-Gerontology board certification, Dr. Dhanani brings an advanced understanding of how the brain, metabolism, and cardiovascular system influence everyday health. That training supports careful diagnosis, individualized treatment plans, and proactive management of conditions like diabetes, hypertension, migraines, cognitive changes, and chronic pain through secure telehealth care.
